In general, lowercase professional designations:
certified kitchen designer
certified diabetes educator
certified public accountant

However, capitalize the associated acronyms:

Exceptions to the lowercase rule:
Realtor (because the term is trademarked)
Master Gardener (to avoid confusion with master as an adjective meaning skilled)
